Wanted to say 'the indication must be carefully established', but the client is querying this.

There can be an indication FOR a certain treatment, but not an indication OF a certain treatment

so you could say

the indication for it must therefore be carefully established/considered

indication I don't think it's necessary to leave in "indication", but your suggestion "when the use of a beta-blocker is indicated .. " is feasible

The problem with the rejection of the word indication it that it actually refers to the indication for prescribing the betablocker. In medical terms an indication or indictions is the ground or grounds on which the medicine is prescribed. In the case of hyperthryoidism the indication is that the thyroid is failing to produce thryoid hormone.
Perhaps you can leave in 'indication' but rewrite the sentence, for instance 'When use of a betablokker is indicated, this use must be carefully considered as it can cause worsening of psoriasis.'

neutral Barend van Zadelhoff : I don't think you can say: the indication of a beta-blocker (its indication) must be carefully considered, can you? "the indication of a beta-blocker??" I don't think it's correct, is it? You can say: "a beta-blocker is indicated" though

You can't use "indication" here, this is a "false friend"

considering a course of treatment

I think that 'indicatie' translates somewhere between symptoms and treatment in English. I think the term above seems to capture that well.
Example sentence:

Exacerbation of psoriasis is a common side effect of beta blockers; this course of treatment should therefore be considered carefully before being prescribed.

Tijdens gebruik van een betablokker kan psoriasis mogelijk verergeren; daarom moet de indicatie zorgvuldig worden gesteld.
