Authors of this kind of reports sometimes use "O" to mean "0":

However, this most probably does not apply to this case as "UPT" is preceded by "1x". On the other hand, "D" alone does not seem to be enough to denote any particular type of induced abortion, it could nevertheless perhaps stand for "at home" (i.e. the abortion pill) as opposed to surgical methods performed at clinics (e.g. D&C, D&E).
To be more precise, the term "potrat" means "abortion", i.e. both a spontaneous abortion/miscarriage (this case since specified as such) and an induced/voluntary one. "Ab" (abort/abortus) is used in the report to mean "induced/voluntary termination of pregnancy".

So, it would be something like Gynecological History: Menarche at 12 years old, 2 spontaneous pregnancies, 1 previous negative pregnancy test, previous miscarriage in 2016.
