Dutch term

co/d

Dutch to English Medical Medical: Health Care abbreviation for prescribing medication
Alprazolam 0.5 mg, 1x 0.5 co/d

This is written on a Dutch treatment plan for a hospital patient and also occurs in isolation as 'co' in relation to a bridging plan for the drug Marcoumar (anticoagulant). I assume it means something like PRN in English but I can't find any evidence for this.

The other part of your question... ...probably pertains to the follow up by the 'co' as in coassistent, but without more context I can't be sure. Please see def. 2: http://www.woorden.org/woord/co
